Tsukai does indeed translate to 'familiar spirit', but in western context a familiar spirit is akin to a daemon, a supernatural intermediary between a god's will and mortals. So calling Ooishi as Oyashiro's Familiar isn't wrong, but it muddies the waters in English (especially if you are trying to imply Umineko is happening in Higuashi). That's one of the reasons why it is oftentimes translated to Oyashiro-sama's 'messenger' or 'servant'. Calling Ooishi a familiar implies that the power is originating with him & not Oyashiro")
