There's a pretty bad mistranslation this episode, when Mikoto is lamenting over Kongou getting hurt in the hospital. The way you're translating it sounds like Mikoto doesn't think Misaki attacked Kongou and believes it's part of a fight between factions. The issue here however is that the fight between factions Mikoto is referring to is between her and Misaki, as Misaki has historically been opposed to Mikoto gaining a faction of Tokiwadai students of her own (to mirror herself) and has messed with her to put a stop to any event that could lead to this. Furthermore at this point in time Mikoto still fully suspects Misaki is behind everything that's happened so far, as you can see by her acting so cold to Misaki's guards when leaving the hospital and later when she lets Baba go, thinking he had been controlled by Misaki.
What that scene is supposed to convey is the fact that Mikoto is shocked that Misaki would so far as to attack Kongou. The correct translation would be that she thought this was merely part of Misaki's regular faction fight with her, and that she thought Misaki would never go as far as to attack another student, let alone a Tokiwadai student like Kongou. Not that she was sure of this.
The way you're translating it sounds like Mikoto is convinced this wasn't Misaki's work and the result of a fight between factions of the underground, while the actual implication here is the complete opposite, namely that Mikoto is shocked to find out Misaki would go this far, as she never thought Misaki would commit such horrors as an extension of their usual (faction) fight.
