PS. What is different with this one episode that it won’t work with K-ite codec Pack & MPHC (black screen at the beginning & no subtitles)?

You should not be using K-lite codec pack, it does not play fansubs well.

Try mpv or install xysubfilter to your mpc-hc and switch to that at least.

Check the CRC of the file you downloaded. Use RapidCRC, or I think “Anime Checker” still works.

If it doesn’t match the filename, the file didn’t download properly.

Well that is just completely false. I have exclusively used K-Lite for the last decade or so over hundreds of shows and can’t think of a single time that it did not play fansubs correctly.
